7/2/2023 EnchantaVaughn Approved QMHPA arrived at the home to conduct individual session with Ms. Broadnax. Ms. Broadnax appeared to be in a positive mood today evident by having a positive disposition. QMHPA informed her that the session on her feelings and anxiety and depression and learning how to express feelings. Upon arrival, she did not present any symptoms of S/I. QMHPA begin session by inquiring about Ms. Broadnax's overall mental health stability. QMHPA recapped from the last session. QMHPA utilized open discussion with Ms. Broadnax to have her express concerns and anxieties. Ms. Broadnax was taught basic concepts of how to identify and verbalize her emotional needs. QMHPA assisted Ms. Broadnax with contributing factors that triggers underlying feelings of depression. Ms. Broadnax's distorted negative beliefs about herself were reframed as she have negative thoughts about herself. QMHPA gave Ms. Broadnax tentative examples of how her distorted thinking controls her day to day functions and how she thinks about situations, people, and things. QMHPA reviewed journaling, and listening to music to help problem solving and conflicting behaviors. QMHPA challenged the Ms. Broadnax to face uncomfortable situations using coping skills to build confidence about herself. Ms. Broadnax was helped to identify the negative cognitive messages that feed her fear of rejection and failure. Ms. Broadnax was receptive to feedback and information provided QMHPA. Ms. Broadnax was open to QMHPA's suggestions for today's session. Ms. Broadnax reported that she didn't realize that she was in her own way as it relates to her thoughts and feelings about people, things, and situations. She reported that she has been hurt for so long, been mistaking as a man on a daily basis and struggles with herself image as these are contributing factors to most of her depressive feelings. Ms. Broadnax was helped to realize that her frequent remarks about herself impede on building positive thinking and building her confidence. Ms. Broadnax admitted that she over thinks a lot to things by analyzing her own thoughts.
